AMC1 Article 15(1)(d) Conditions for obtaining a certificate ED Decision 2022/022/R OCCURRENCE REPORTING The applicant should establish procedures for reporting occurrences to the competent authority and to any other organisation necessary, which should include a description of: (a) the applicable requirements for reporting; (b) the reporting mechanism, including reporting forms, means, and deadlines; (c) the personnel responsible for reporting; and (d) the mechanism for identifying root causes, and the actions to be taken to prevent similar occurrences in the future, as appropriate.
