GM1 21.A.3A(a) and 21.A.3A(b) Reporting system ED Decision 2022/021/R GENERAL — COLLECTING SYSTEM The term ‘collection’ means the setting up of systems and procedures that will enable relevant failures, malfunctions, and defects, or other occurrences, to be properly collected when they occur. As the collection system needs to accept reports that originate outside the organisation (from operators, maintenance organisations, suppliers, etc.), it is necessary to inform possible reporters of the existence of the system and of the appropriate means to introduce reports into it. This does not presume that direct access to the system is to be granted if other mechanisms are more appropriate. The collection system should also ensure the collection, through an internal reporting scheme, of internal errors, near misses, and hazards that are perceived by the reporter as an actual or potential aviation safety risk. Considerations for the collection of information related to events should include the following: — the analysis of failure rates; — the early rejection of parts from service; and — comparison with the certification assumptions.
