APPLICATION FORM
An applicant for a POA should complete and submit to the competent authority an EASAĀ FormĀ 50 (see below).
The completed form, an outline of the production organisation exposition, and details of the proposed terms of approval should be forwarded to the competent authority.

EASA Form 50
BlockĀ 1: The name of the organisation should be entered as stated in the register of the National Companies Registration Office. For the initial application, a copy of the entry in the register of the National Companies Registration Office should be provided to the competent authority.
BlockĀ 2: State the trade name by which the organisation is known to the public if it is different from the information given in Block 1. The logo of the organisation may be used in this block.
BlockĀ 3: State all the locations for which the approval is applied for. Only those locations should be stated that are directly under the control of the legal entity stated in BlockĀ 1.
BlockĀ 4: This block should include further details of the activities under the approval for the addresses indicated in BlockĀ 3 The āGeneralā block must include overall information, while the āScope of approvalā block should address the scope of work and the products/categories following the principles laid down in GMĀ 21.A.151. The āNature of privilegesā block should indicate the requested privileges as defined in pointsĀ 21.A.163(b)-(e). For an application for renewal, state ānot applicableā.
BlockĀ 5: This block should provide a summary of the organisation with reference to the outline of the production organisation exposition, including the organisational structure, functions and responsibilities. The nomination of the responsible managers in accordance with pointĀ 21.A.145(c)(2) should be included as far as possible. For an application for renewal, state ānot applicableā.
BlockĀ 6: The information entered here is essential for the evaluation of the eligibility of the application. Therefore, special attention should be given concerning the completion of this block, either directly or by reference to supporting documentation, in relation to the requirements of pointsĀ 21.A.133(b) and (c) and the AMC toĀ 21.A.133(b) andĀ (c).
BlockĀ 7: The information to be entered here should reflect the number of staff, or in the case of an initial approval, the intended number of staff for the complete set of activities that are to be covered by the approval, and it should also therefore include any associated administrative staff.
BlockĀ 8: State the position and name of the accountable manager.
